Danielle DiGennaro MSN, ANP-BC

Agrace HospiceCare

Danielle is an adult NP who started her career in advanced heart failure and transplant at Duke University Medical Center as an RN and later as an NP. Following a cross-country move, to Oakland, CA, she worked in Palliative Care for Sutter Health (Palo Alto Medical Foundation). She was drawn to palliative care out of an interest in learning about patient's access to medical care in rural areas or healthcare deserts following being declined for intervention at major medical centers. While in CA, Danielle also worked for Hope Hospice, proving after hour call support. Another cross-country move brought Danielle and her family to Gainesville, FL where she worked for Haven Hospice, originally in hospice, then developing their palliative care program. Haven ultimately had a successful inpatient palliative consult service at the local HCA hospital with a growing outpatient home based palliative care program. During her time in FL she also worked in an embedded palliative care clinic in oncology through University of Florida Shands. In part due to demand and Danielle's background, she also opened an embedded palliative care clinic in the advanced heart failure clinic. The most recent move to the Midwest had Danielle join Agrace HospiceCare in Madison, Wisconsin where she serves as the director of supportive (palliative) care.
