Hear from two institutions where successful heart failure remote monitoring programs have been implemented and proven sustainable and scalable. This webinar will review the specific, replicable steps essential for implementing a heart failure monitoring program, including stakeholder buy-in, staffing models, workflows, billing compliance, and revenue considerations. The discussion will also explore available diagnostic data from Cardiac Implantable Electronic Devices (CIEDs), with a look at other data streams such as pulmonary artery pressure monitoring and remote blood pressure, weight, and pulse oximetry measurements.
